Solidaris Brabant geniet het vertrouwen van meer dan 645.000 leden. Om de dienstverlening van de sociale dienst te optimaliseren zoeken wij, in het kader van een contract voor onbepaalde duur, voor spoedige aanwerving een (m/v/x)* Indicatiesteller Sociale Dienst / Belrai-expert Je werkt als indicatiesteller voor de Dienst Maatschappelijk Werk (DMW) van Solidaris Brabant. Je maakt deel uit van een klein team van indicatiestellers binnen onze DMW enerzijds, en je werkt samen met zelfstandige indicatiestellers anderzijds. Je neemt z orgzwaarte-metingen (of “indicatiestellingen”) af in het thuismilieu van onze cliënten met het oog op een aanvraag van het zorgbudget voor ouderen met een zorgnood en het zorgbudget voor zwaar zorgbehoevenden. Dit gebeurt aan de hand van de Belrai-screener. Je bouwt een e xpertise u i t met betrekking tot de zorgzwaarte-metingen aan de hand van Belrai en je ondersteunt een team van zelfstandige indicatiestellers waar we mee samenwerken voor het uitvoeren van ‘zorgzwaarte-metingen'. Je kan via ons de opleiding tot Belrai-trainer volgen. Zo word je voor hen een aanspreekpunt: je organiseert opleiding en intervisie voor onze indicatiestellers omtrent het werken met de Belrai-Screener en je ondersteunt hen in hun dossiers indien nodig. Je staat – samen met een administratieve medewerker – in voor de administratieve o pvolging van deze aanvragen tot zorgzwaarte-meting. Dit houdt in dat je mee de nodige behandelingstermijnen opvolgt en kan inspringen waar nodig. Je bent ook onze back-up voor andere administratieve taken zoals de facturatie. Je maakt in het kader van de huisbezoeken verplaatsingen in ons werkingsgebied Vlaams-Brabant en Brussel. De focus ligt daarbij op Halle-Vilvoorde, voor de andere regio's ben je een back-up. Voor de verplaatsingen kan je gebruik maken van een dienstwagen. Je werkt voor het administratieve luik in ons hoofdkantoor in Brussel (Zuidstraat 111), eventueel aangevuld met gedeeltelijk thuiswerk. Diploma: Bachelor in het studiegebied sociaal-agogisch werk (Maatschappelijk Werk, Orthopedagogie, Gezinswetenschappen, …) Bachelor in het studiegebied gezondheidszorg (Ergotherapie, Sociaal Verpleegkundige, …); Goede kennis van het Fr ans , zowel mondeling als schriftelijk; Ervaring binnen de welzijns- of gezondheidssector is een pluspunt; Kennis van zorgzwaarte-meting (BelRAI, of andere instrumenten) is een grote Je bent in het bezit van een rijbewijs type B en kan vlot overweg met MS Office Vaardigheden Je bent klantgericht, discreet, diplomatisch, luistervaardig, planmatig en organisatorisch sterk. Je kan je vlot mondeling en schriftelijk uitdrukken; Je kan zelfstandig werken, maar functioneert goed binnen een team; Je bent leergierig en je bent bereid om je bij te scholen; Je bent flexibel en je bent bereid om je te verplaatsen (voor professionele verplaatsingen is een dienstwagen ter beschikking).
