Role:
Speech Language Pathologist working with older adults in private homes and senior living communities, delivering personalized, evidence-based therapy aligned with the Geriatric House Calls model.
Key Responsibilities
- Perform evaluations, develop individualized treatment plans, and provide therapy to promote patient wellness, mobility, and independence.
- Deliver one-on-one care in various settings, including virtual care as appropriate.
- Document clinical activities accurately within electronic medical records.
- Collaborate with interdisciplinary teams to support comprehensive patient care.
- Educate patients, caregivers, and staff on safety, function, and aging-in-place strategies.
Qualifications & Skills
- Valid Speech Language Pathology license or eligibility in practice state(s).
- Degree from an accredited SLP program.
- Proficiency in computer use and EMR systems.
- Current CPR certification.
Physical Requirements:
Ability to perform moderate physical activities including lifting, repositioning patients, and driving between locations, with necessary use of PPE.
