Part-Time Social Worker: Care Coordination

Encore Healthcare Services is seeking a part-time Social Worker to identify and address the social and emotional needs of long-term care residents. You will assist with arranging services, and act as a liaison among residents, families, staff and the community.

You will visit residents, prepare documentation, participate in care plan meetings and coordinate discharge planning as needed, providing ongoing support to residents and families.
