Location of Post: CDNT6 - Palmerstown

Hours of Work: 35 hours per week

Job Purpose

The post holder will be responsible for the provision of quality evidence based Social Work service including assessment of family needs, and a range of advisory, practical and skilled emotional supports to children and young people 0 to 18 years and their families accessing the Children’s Disability Network Team. The service is provided within the Progressing Disabilities Services for Children and young people (PDS) framework, with interdisciplinary team working and family centred practice at its core.

The duties of this post will include direct intervention with children and families and having a social work role in particular team projects or developments as directed by the CDNT Manager.

Reporting Relationship: The Social Worker will report to the Children’s Disability Network Manager

Principal Duties and Responsibilities

Planning and Organising of Activities and Resources

Plans and organises activities and necessary resources efficiently and effectively.

Understands the importance of time management for themselves and in particular how this translates to organising their assigned caseload.

Demonstrates the ability to prioritise and arrange their caseload and workload accordingly.

Deals with unexpected scenarios under supervision with persistence and flexibility to ensure the achievement of goals.

Maintains a disciplined and professional level of performance under sustained or situational pressure with support from Senior Social worker and CDNM.

Incorporate Social Work values and ethical principles in planning, developing, implementing and reviewing interventions.

Professional Development and Standards in the Service

To provide a range of Social Work assessments and interventions to children, young people and their families accessing the service.

To conduct social work assessments.

To provide ongoing social work support to designated families on an individual and group basis

To liaise with HSE in the co-ordination of respite and home support services.

To attend relevant Tusla child protection case conferences.

To provide training to family members and young people as determined by the teams in conjunction with the CDNM and other team members e.g. Early Bird, Parents Plus, and Sibshops.

To provide training to external agencies as required in agreement with the CDNM.

To promote and deliver services within the ethos and philosophy of Stewarts Care.

To monitor and keep up-to-date with developments in Social Work including attending training courses and participating in evaluation of the Service and research initiatives.

To be involved in relevant Social work aspects of policy development within the CDNT.

Is adequately aware of all Stewarts Care and Progressing Disability Policies and standards, legislative and professional requirements to ensure appropriate standards in their area of responsibility.

Ensures that all records and technical data are up to date and available if required.

Service Delivery & Initiative

Collaborate with the child / young person while working in partnership with their parent/family/carer and all other relevant stakeholders to identify and achieve goals through the Individual Family Support Plan meetings.

Provide supportive counselling, emotional and practical support, and information to service users and their families.

Takes initiatives to move the service forward and shows a willingness to try out new ideas under supervision that add service delivery value.

Is committed to achieving evidence-based goals and the continuous improvement of the service.

Shows enthusiasm and a high level of motivation in completing projects.

Team Working

To work in collaboration with the other members of the interdisciplinary team in the planning and delivery of services for children and young people, in particular in the development of child and family centred planning.
