Functieomschrijving

Als return to work coördinator ondersteun en oriënteer je leden die langdurig ziek zijn, bij het zoeken naar werk. Je zal op een neutrale wijze de vraag en noden van deze personen afstemmen op het dienstenaanbod van de maatschappij voor een duurzame herinschakeling van langdurig zieken in de arbeidsmarkt. Hiervoor werk je nauw samen met het multidisciplinair team van de betrokken regio waarvan je deel uitmaakt, meer bepaald de adviserend arts en de referentie-paramedicus.

Welke uitdagingen staan je te wachten?
Je denkt mee na over het nieuwe beleid ter bevordering van de beroepsherinschakeling van begunstigden van een vervangingsinkomen. Je neemt deel aan intervisies met zijn/haar collega’s-coördinatoren in andere regio’s. Je bouwt mee een netwerk van regionale partners op om aan de noden van de leden en het personeel van het Medisch departement te beantwoorden.

Belangrijkste opdrachten

Begeleiding en ondersteuning van de leden
- Gesprekken voeren met de leden (op vraag van de adviserend arts / op initiatief van de leden).
- Inlichtingen verstrekken aan de leden en ze begeleiden bij vragen over werkhervatting en de mogelijkheden daaromtrent.
- Leden in arbeidsongeschiktheid proactief opvolgen gedurende een langere periode (+10 weken).
- Indien nodig leden helpen bij het invullen van de vragenlijst QMV.
- Vragen en noden beluisteren van het lid en ze proberen te begrijpen, vanuit een neutraal, onpartijdig standpunt.
- Het actieplan in het werkhervattingsdossier afstemmen op het arbeidsongeschiktheidstraject (dat de adviserend arts uitwerkte).
- De leden in contact brengen met organisaties / dienstenverleners die een antwoord kunnen geven op hun vragen en noden.

Opvolging en verantwoordelijkheid voor het dossier
- Opmaken van een Terug Naar Werk-dossier aan en nagaan of dit de documentatie bevat die de procedure vereist.
- Opvolging van het dossier.
- De verschillende nodige stappen volgen bij het werkhervattingsproces en het resultaat van het traject documenteren.
- Aandacht hebben voor de procedures bij het Terug Naar Werk-proces en ze nauwgezet opvolgen.

Opbouw van netwerk en deskundigheid
- Een netwerk bouwen en onderhouden in verband met de Terug Naar Werk
- dossiers voor een goede en efficiënte heroriëntering.
- De Terug Naar Werk
- dossiers intern en extern coördineren.
- Deelnemen aan overlegmomenten tussen de Return to work - coördinatoren van de verschillende interdisciplinaire teams over de regio's heen.
- Regelmatig deelnemen (meermaals per jaar) aan intervisies met het RIZIV.
- Onvolmaaktheden of problemen in de werking melden aan de stafmedewerkers om zo naar een optimale werking te streven?
- Kennis onderhouden over het themaTerug Naar Werk, gezondheidszorg, KATZ en KAPPA, om onze leden steeds goed te kunnen informeren.

Profiel
- Je beschikt over een bachelor in een sociaal-medische richting.
- Je beschikt over een diploma Terug Naar Werk - coördinator (diploma in bezit of te behalen binnen de twee jaar).
- Je hebt relevante beroepservaring.
- Je hebt een grondige kennis van de sociale landkaart, werkhervatting en vrijwilligersorganisaties,....
- Je bent communicatief sterk en sociaal vaardig waardoor je vlot samenwerkt, wendbaar bent en je je gemakkelijk aanpast aan je gesprekspartners.
- Je werkt op een natuurlijke en spontane manier met je partners en je kan een netwerk opzetten.
- Je volgt op of de doelstellingen worden bereikt en je werkt aan een continue dienstverlening.
- Je onderscheidt je door je proactieve houding en ziet verbeteropportuniteiten die je concreet maakt.
- Je behoudt een transversaal overzicht.
- Je werkt zorgvuldig en nauwkeurig in opvolging van jouw dossiers.
- Je bent goed in plannen en organiseren.
- Je bent flexibel en kan je aanpassen in een dynamische context van verandering.
- Je werkt in eerste instantie in regio Sint-Niklaas maar bent flexibel inzetbaar in een ruimere regio binnen de provincie Oost-Vlaanderen.
