VOA - Puerto Rico

Volunteers of America Puerto Rico is expanding the capacity of its Community Health Worker model to improve access to health and social resources for underserved populations. The program uses community-based approaches to address barriers to care and social determinants of health while strengthening connections between individuals, healthcare providers, and community resources. The Community Health Training & Performance VISTA will build sustainable infrastructure for Community Health Worker training, onboarding, performance measurement, data collection, and continuous quality improvement. The member will develop tools and systems that allow VOAPR to strengthen consistency, measure impact, and replicate effective community health practices. This is a capacity-building assignment rather than a direct-service CHW position. The VISTA will create resources and systems that strengthen the work of VOAPR staff, Community Health Workers, partners, and future members. Further help on this page can be found by clicking here . Member Duties : Assess existing Community Health Worker training, onboarding, educational resources, data collection processes, and performance measurement systems. Research evidence-informed CHW training and quality improvement practices. Develop standardized training materials, onboarding resources, referral education, and Train-the-Trainer tools. Develop performance measurement and data collection resources and strengthen CommCare workflows for documenting community health activities and outcomes. Establish baseline measures and develop a Continuous Quality Improvement framework. Gather feedback, evaluate tools and systems, document procedures, and develop recommendations for future implementation and expansion. Program
