About The Role
We are looking for a Community Health Officer (Health Management) with expertise in exercise for weight management, physical function, and sarcopenia prevention (肌少症). You will work directly with working poor families, building trust and supporting community members who may face challenges in managing their health. Working closely with our multidisciplinary team, including teammates who specialise in healthy eating and nutrition, you will turn professional knowledge into practical, sustainable lifestyle changes, helping people improve their physical wellbeing, stay active, and take greater ownership of their health. This is a hands-on community role for someone passionate about exercise, preventive health, and making a meaningful difference in grassroots communities.
Key Responsibilities
Community Programmes & Service Delivery Conduct community health assessments, screenings, and follow-up. Support health workshops, cooking classes, exercise classes, and other community programmes. Coordinate appointments, enquiries, referrals, and follow-up. Maintain service records and statistics for monitoring and reporting. Work with schools, restaurants, workplaces, and community partners to promote healthy living. Support other programme initiatives as needed. Community Engagement & Empowerment Engage working poor families, including those with low health awareness or health ownership. Build trust and motivate community members to take charge of their health. Co-create practical and sustainable healthy lifestyle solutions with community members. Communicate effectively with service users, caregivers, and community partners. Exercise & Health Management Design and lead exercise programmes for weight management, physical function, healthy ageing, and sarcopenia prevention (肌少症). Assess participants’ needs and provide tailored exercise and lifestyle guidance. Deliver practical health education and develop accessible health materials.
