Trajectory Development Coordinator – Contract

Position Description Funded by The Hong Kong Jockey Club Charities Trust, the TDC coordinates community-based early intervention services for young mothers and their children. Working with families, a multidisciplinary team, and community partners, the TDC delivers strengths-based, evidence-informed interventions that promote maternal well-being, responsive parenting, child development, and family stability. Job Responsibilities Coordinate with the Early Intervention Services (EIS) team (Occupational Therapist, Speech Therapist, and Registered Nurse) to deliver interventions under the Fulfilling Parenthood Framework (FPF) and Positive Trajectory Framework (PTF). Facilitate the development, implementation, and review of Individualised Development Plans tailored to each family's strengths, needs, and aspirations. Coordinate with the EIS team to deliver parenting training on responsive caregiving and developmental milestones, and drive monitoring of children's progress, referrals, and follow-up when red flags are identified. Engage fathers, partners, extended family, and community networks to strengthen family support. Chair multidisciplinary team meetings and collaborate with internal teams, community centres, and external partners to ensure coordinated case planning and service delivery. Oversee risk escalation and safeguarding protocols, ensuring timely and appropriate responses when concerns arise for young mothers, children, or families. In close partnership with the Programme Officer, track programme outputs and outcomes, maintain accurate records, and prepare timely data and reports for the Trust and The Hong Kong Polytechnic University research programme. Monitor project budget utilisation, flag financial concerns or variances to the relevant manager, and support expenditure documentation, grant compliance, and audit preparation. Travel on a rotational basis to designated community centres across Kowloon and the New Territories to deliver services and training. Undertake any other duties as assigned in support of programme and departmental objectives. Job Requirements Bachelor's degree or above in Social Work or a related discipline. Registration with the Hong Kong Social Workers Registration Board is preferred. Equivalent professional qualifications are welcome. Minimum 3 years of experience in social services, family support, or child development, including work with young mothers, infants, or families in vulnerable circumstances; prior experience on Hong Kong Jockey Club Charities Trust-funded projects is a significant advantage. Genuine interest in early childhood developmental milestones, and parenting training, with ability to build competency in these areas and facilitate group-based learning programmes. Able to assess needs, coordinate interventions, manage risk, and maintain accurate documentation, including familiarity with funder reporting, grant compliance, data management, and audit preparation. Strong interpersonal and communication skills with the ability to build trust across diverse stakeholders and collaborate effectively within a multidisciplinary team. Organized, self-motivated, and able to manage multiple priorities both independently and as part of a team, with flexibility to work across community-based and varied settings. Good command of written and spoken Chinese and English. Must share the commitment of Mother’s Choice in upholding the safety and protection of children as paramount.
