**Position**: Executive, Communities of Care
**Overview**:
- Reporting to the Manager of Communities of Care, the person will:
- Together with AIC and community partners, support **the implementation of the**
**approved workflow for LB AACs to improve the referral process and building a**
stronger network and partnerships with key service domains in the local region.
- **Deepen and/or broaden**the partnerships in 9 baseline health-social services as
identified by AIC within defined service boundary.
- **Support in development of structured collaborations and national protocols**
that can be applied as a standard for the sector.
- **Harmonise the best practices and learnings into a framework**to support other
providers in implementing CoCs
**Job responsibilities**
- Work as a team in either lead or support the creation, planning and implementation of
workflow, programmes and activities to support the objectives of COC and ensure that they
meet the programme objectives and performance indicators (KPI).
- Perform outreach to the seniors within service boundary.
- Perform community profiling and identify the needs, interests and concerns of residents
through methods including but not limited to door-knocking, surveys, roadshows etc.
- Engage with relevant stakeholders such as other Community Care service providers,
community partners and government agencies to develop and pilot workflows, protocols
and process that facilitate the integration of social and health care for residents under
population health model.
- Ensure that workflow is communicated to all relevant parties including the seniors, monitor
and adjust when necessary.
- Conduct trials of the processes, collect feedback and review efficiacy with service partners
for improvement and refinement.
- Assist the Project Head/Leader with project and programme reporting requirements and in
maintaining the database.
- Identify, stock take and assess the strengths and limitations of community assets, including
other Community Care service providers.
**Requirements**:
- Degree/Diploma holder with experience in social service sector
- Minimum 1 year experience in Project Management/relationship management or related
- Experience in Social service sector will be an advantage.
- Possess excellent interpersonal skills, meticulous and task orientated.
- A team player & equally adept at working independently.
